New this year USA Water Polo has implemented a procedure were athletes must have proof of identification AND proof of birth date.  Each coach must present a binder with this information for each roster and team that participates in Quals and then in JOs.

Although the rosters are not yet posted, athletes who are in consideration to be on a JO Quals or final JO roster, please start to get these forms together.  We will notify you when and where to turn them in.

Please bring copies, not original documents.

* Birth certificate works for age but not for ID verification
* USA Water Polo card with picture ID is good for ID but not age
* Passport works for both
* Driver’s license works for both
* School ID works for ID verification but not age
